Population health leaders come from multiple sectors and professions, but we share a common goal: to advance well-being and health equity for individuals and communities. At the 2019 ACHI National Conference, we will come together as a community of population health leaders to amplify our shared commitment and message with One Voice. By speaking and acting with one voice, we can activate partnerships to drive community and organizational transformation, becoming a unified force for health equity and well-being in our communities. The Association for Community Health Improvement, an affiliate of the American Hospital Association, is the premier national association for community health, community benefit and population health professionals.
