At the Vancouver Housing Authority (VHA) we work closely with local governments and other community partners throughout Clark County to address issues of affordable housing and homelessness and help families break the cycle of poverty. We provide affordable housing and housing assistance to low-income residents in Clark County. The Supportive Housing Service Coordinator 1 works with VHA residents to assess service needs, determines eligibility for public services and provides referral assistance and links them to supportive services provided by public agencies or private practitioners within the general community.
Your skills and experience will support our mission as you:
Provide general care coordination including intake and referral services to all residents needing such assistance.
Establish connections with appropriate agencies and service providers in the community.
Set up a directory of providers for use by both staff and residents.
Refer and link residents to applicable service providers in the general community (e.g. case management, personal assistance, transportation, counseling, preventive health screening/wellness, parenting classes, credit counseling, and legal advocacy).
Educate residents on service availability, application procedures, resident rights, etc. Provides advocacy as appropriate.
Monitor the ongoing provision of services from community agencies and keeps the case management and provider agency current regarding the progress of the individual. Manages the provision of supportive service where appropriate.
Help the residents build informal support networks with volunteers and/or other residents, family and friends.
Provide training to residents in the obligations of tenancy or coordinate such training.
Educate other staff on the issues related to area of specialty to help them to better work with and assist the residents.
Pay and Benefits
The starting salary range for this position is $3,509 - $3,860 per month based on experience. The full salary range for this position is $3,509 - $5,008 per month.
Full-time, non-exempt, union-represented position working 40 hours per week, Monday - Friday.

Requirements
Bachelor's Degree in Social Service preferred.
2 years of direct service experience working with senior citizens and nonelderly disabled individuals required.
Experience and/or training in the aging process, elder services, disability services; determining eligibility for and procedures of Federal and applicable State entitlement programs; legal liability issues relating to providing services coordination, drug and alcohol use and abuse by the elderly, as well as mental and cognitive health issues.
Valid driver’s license and fewer than 3 moving violations within the last 3 years.
